A 62-year-old patient was admitted with suspected appendicitis. On the preoperative CT scan, a 4 × 6 × 8 cm lipoma was found with suspicion of invagination of part of the ileum. A right hemicolectomy was done; the lipoma was ulcerated at the apex which explained the symptoms. The patient had an uneventful recovery and was discharged on the 5th postoperative day.
