A 50-year-old man presented with acute appendicitis. At operation, the appendix was inflamed, but multiple small focal lesions were also found on the antimesenteric border of the distal half of the small bowel (figure). A full-thickness small bowel biopsy and appendicectomy were performed. Histology confirmed multifocal carcinoid tumour and appendicitis. The patient went on to have all macroscopic disease, involving over two-thirds of the small bowel, resected, including the mesenteric lymph nodes. At 2-year follow-up, the patient is disease-free.
