This patient had a laparotomy for bowel obstruction, as a gallstone is obstructing the small bowel.

Gallstone ileus is managed by laparatomy and stone extraction via a proximal enterotomy. The remaining small bowel must be milked for further gallstones. The gallbladder has typically formed a fistulous tract to the duodenum and is managed conservatively.
