A 32-year-old man was referred with a short history of the illustrated finger changes and a swelling in the same arm at the site of a previously ligated arteriovenous fistula. Clinical examination showed splinter haemorrhages in the right hand. The pulsatile swelling in the upper arm, as demonstrated on computed tomography reconstruction, is a partially thrombosed pseudoaneuysm arising from the brachial artery. Surgical repair of the pseudoaneurysm was performed and at 3- and 6-month follow-up the nails had returned to normal.
