This is a right colonic volvulus. A 66-year-old man presented with a 2-day history of generalized abdominal pain and constipation. Physical examination revealed a distended abdomen with peritoneal signs on the left. At operation he had a distended ascending colon, which was twice twisted clockwise 10cm proximal to the hepatic flexure. Right hemicolectomy and ileocolic anastomosis were performed; postoperative recovery was uneventful.
