This is the image of a laparoscopic cholecystectomy performed on a 46-year-old man with upper abdominal pain and gallstones demonstrated on an abdominal ultrasound scan. The patient had an anatomical variation: an intrahepatic gallbladder. A hepatotomy was done to access the gallbladder, and intraoperative cholangiography confirmed biliary tree anatomy. The gallbladder is formed during the first 3 weeks of fetal development and becomes extrahepatic after 8 weeks. Usually, an intrahepatic gallbladder is dysfunctional and the association with gallstones is frequent.
