The appendix is wrapping around a portion of small bowel and its mesentery causing small bowel obstruction. This has previously been described as an appendicular tourniquet. The appendix and the small bowel it adhered to were resected. Histology revealed an appendicular-enteric fistula, likely from appendiceal diverticular disease, with no evidence of malignancy.
