Stepwise management of a large haemorrhagic walled-off necrosis involved insertion of a large-bore percutaneous drain followed by endoscopic ultrasonography and internal drainage using the multiple transluminal gateway technique. Percutaneous necrosectomy was later performed, with excellent clinical outcome.
