Conclusion

All 92 patients in the surgery arm underwent uneventful surgery. Thirty-seven patients from the medical group crossed over to surgery because of new symptoms, with no postoperative deaths or strokes. Late hemispheric and retinal symptoms were lower in the surgical group (7.6 vs. 21.1 per cent, P=0.01).
