Conclusion

Some 480 patients with a midline laparotomy were randomized to suture repair, or augmented repair with either sublay or onlay mesh. Clinical outcomes to one month were similar, but more patients who had onlay mesh developed a seroma (18.1 per cent), compared with sublay mesh (13.7 per cent) or primary suture (2.9 per cent); P=0.002.
